Change is inevitable. Organization that does not undergo a change will not survive.In this scenario we critically analysis with the help of Lewis’s framework, the change in our organization that improved the patient’s wellbeing and at the same time enhancement of nurse’s productivity. The change described below is towards Electronic Health Record which is a systemized collection of patients health information in a digital format that can be shared across different health care settings.
What is the proposed change?
In our organization we have an assessment room where patients’ vital signs are checked, a triage form is used to fill these vital signs, it is usually filled by hand and therefore errors occurred many times due to handwriting issues. The real time experience on failure of proper communication via triage form led to the proposal of Electronic Health Record (EHR) in outpatient department. The patient’s height and weight were interpreted wrongly, as a result there was difference in Body Mass Index (BMI) and there by the patient was given overdose medication at the time of Surgery. Based on this incident report the proposal of EHR in Outpatient department was initiated by me with the in-charge. This stage is called the unfreezing stage.
How will the goal of the change be explained/ described? How will you challenge the status quo?
The goal is to help healthcare professionals use quality data for decision-making in patient care by evidence-based knowledge management (4).Doctors to have direct access to patient’s assessment, and to reduce the waiting time of patient by entering details digitally. The challenges are introduction of EHR needs investment by management and appropriate training for the staff.
Who are the key stakeholders – those who would be impacted by the change?
People and system are always impacted by the change. The initiation of EHR will have an impact on the nurses and doctors who are the key stakeholders. EHR facility will speed up the assessment process and can save manpower in the assessment room there by benefiting management through cost saving. The EHR system minimizes the chance of error in entering vital signs because of its error detective system. This is the stage of change.
How everyone (key stakeholders) who needed to be involved was included in the process?
Nurses who are basically the primary change initiators were involved in the process along with doctors who are benefited by getting the patient assessment on their desktop immediately upon patient arrival at the assessment room. Management was also involved in the process of change by providing the facilities for the change and by arranging adequate training for the staffs. The success of EHR system implementation process is based on the commitment and involvement of all stake holders. System becomes a failure if there is lack of support and negative attitude from the nursing and administrative community (1).
What are the actions that you would utilize to implement the new initiative?
Changes are not always accepted by the people therefore proper planning and actions need to be initiated among nurses and doctors to implement the change which includes new system awareness program. The usability of EHR system is the key factor while designing and implementing the system which fits to the workflow of nurses as well as provides patients information clearly (2)..
What are the primary barriers to the change and how will they be managed?
Head to foot examinations are not done through EHR by all doctors which is considered as a primary barrier. Access to the assessment of patients is limited to the outpatient department therefore Inpatient department have non-access to such patient assessment. Loss of digital data due to system failure is a risk. EHR is a single commercial health information exchange platform to integrate the disparate clinical systems (5).Further discussion to be carried out on EHR for inpatient department.
What are the indicators of successful implementation of your change plan?
This is the refreezing stage where changes are integrated, and sustainability is monitored.The waiting period of patient become less. Now a days the assessment time of patient is only 3 minutes whereas earlier it was 5 minutes. More accurate information are obtaining digitally without any handwriting legibility issue. Doctors are receiving assessment information immediately upon patients visit in assessment room. Lewis’s framework emphasis the importance of workplace culture in the successful implementation of change with in healthcare sector and discusses on the importance of management skills which is required by health professionals (6). Lewis’s framework has a foundational significance in change management (3).
