REFERENCES MUST BE SCHOLARLY WITHIN LAST FIVE YEARS
Sally is a 50-year-old female who has been a jogger for several years. She has recently been diagnosed with osteoarthritis. She has been taking ibuprofen for 3 months but states that “it does not help” and hurts her stomach. The health care provider prescribes celecoxib (Celebrex) 100 mg orally twice a day.
What is the first-line therapy for osteoarthritis and the mechanism of action?
Sally expresses concern about all the recent news about heart problems and celecoxib (Celebrex). What information should be included in a teaching plan to help her understand about taking celecoxib and the benefits and risks?
Ibuprofen and celecoxib are both nonsteroidal anti-inflammatory drugs. Explain how they are similar and different.

Your weekly conversation with your supervisor involves a new manager within you

Your weekly conversation with your supervisor involves a new manager
within your organization. He has been tasked with the formation and
leadership of a team assigned to work on a critical project. Project
success represents potential revenues in excess of five million dollars.
The upfront investment in this project is significant and the stakes
are high, but if it works, the return on investment is worth the initial
cost.
Leadership wants to ensure that the right people are
working on this high-profile project as many departments throughout the
organization will be impacted by it. They have asked your supervisor to
support the new manager as he builds the project team. Leadership is
anticipating justification for team selection before proceeding.
Scenario Questions
Your
supervisor has asked you to use what you are learning about management
to draft guidelines to assist the project manager as he formulates the
team.
Initial Post: Post your recommendations using the following framework:
There
are many factors to consider when putting together a successful team.
In your recommendation, supply your perspective as to the three most
important factors to consider when building this team. Strengthen your
assertion with supporting material from the module readings and from
external resources that you find.
In your opinion, what factors need to be considered when determining the effectiveness of this team?
Thinking about your experience with teams, whether as a leader or member, what were the ingredients for team success?
(Answers to questions A and B should reflect the course readings and should cite that material where appropriate.)

Reply Post: From the point of view of a hiring consultant, provide advice to two of your peers.
Wearing
the hat of the new manager tasked with forming this team, reflect on
the recommendations of your peers. Outline additional insights you have
gained about team building and group dynamics from their posts.
In what ways does their rationale make sense?
Recommend other factors that would strengthen their plan. Include your rationale.
How would you integrate their plan with yours to build a strong team?
To manage your reply, use the following parameters:
Ensure the utility of your recommendations by offering a reply limited to 300 – 400 words maximum.
Incorporate references connecting their details to your reply.
Strengthen the credibility of your recommendations by citing supporting evidence.

Assignment 2: Focused
SOAP Note and Patient Case Presentation

For this Assignment, you will document
information about a patient that you examined during the last 3 weeks, using
the Focused SOAP Note Template provided. You will then use this note to develop
and record a case presentation for this patient. Be sure to incorporate any
feedback you received on your Week 3 and Week 7 case presentations into this
final presentation for the course.
To Prepare
Review the Kaltura Media
Uploader resource in the left-hand navigation of the classroom for help
creating your self-recorded Kaltura video.
Select a child or
adolescent patient that you examined during the last 3 weeks who
presented with a disorder for which you have not already created a Focused
SOAP Note in Weeks 3 or 7. (For instance, if you selected a patient with
anorexia nervosa in Week 7, you must choose a patient with another type of
disorder for this week.)
Create a Focused SOAP Note
on this patient using the template provided in the Learning Resources.
There is also a completed Focused SOAP Note Exemplar provided to serve as
a guide to assignment expectations.
Please Note:
All SOAP notes must be
signed, and each page must be initialed by your Preceptor. Note: Electronic
signatures are not accepted.
When you submit your note,
you should include the complete focused SOAP note as a Word document and
PDF/images of each page that is initialed and signed by your Preceptor.
You must submit your SOAP
note using SafeAssign. Note: If both files are not
received by the due date, faculty will deduct points per the Walden
Grading Policy.
Then, based on your SOAP
note of this patient, develop a video case study presentation. Take time
to practice your presentation before you record.
Include at least five
scholarly resources to support your assessment, diagnosis, and treatment
planning.
Ensure that you have the
appropriate lighting and equipment to record the presentation.

Be succinct in your presentation, and do not exceed 8 minutes. Address the following:
Subjective: What details did the patient provide regarding their personal and medical history? What are their symptoms of concern? How long have they been experiencing them, and what is the severity? How are their symptoms impacting their functioning?
Objective: What observations did you make during the interview and review of systems?
Assessment: What were your differential diagnoses? Provide a minimum of three (3) possible diagnoses. List them from highest to lowest priority. What was your primary diagnosis and why?
Reflection notes: What would you do differently in a similar patient evaluation? Reflect on one social determinant of health according to the HealthyPeople 2030 (you will need to research) as applied to this case in the realm of psychiatry and mental health. As a future advanced provider, what are one health promotion activity and one patient education consideration for this patient for improving health disparities and inequities in the realm of psychiatry and mental health? Demonstrate your critical thinking.

Insomnia is one of the most common medical conditions you will encounter as a PM

Insomnia is one of the most common medical conditions you will encounter as a PMHNP. Insomnia is a common symptom of many mental illnesses, including anxiety, depression, schizophrenia, and ADHD (Abbott, 2016). Various studies have demonstrated the bidirectional relationship between insomnia and mental illness. In fact, about 50% of adults with insomnia have a mental health problem, while up to 90% of adults with depression experience sleep problems (Abbott, 2016). Due to the interconnected psychopathology, it is important that you, as the PMHNP, understand the importance of the effects some psychopharmacologic treatments may have on a patient’s mental health illness and their sleep patterns. Therefore, it is important that you understand and reflect on the evidence-based research in developing treatment plans to recommend proper sleep practices to your patients as well as recommend appropriate psychopharmacologic treatments for optimal health and well-being.

For this Discussion, review the case Learning Resources and the case study excerpt presented. Reflect on the case study excerpt and consider the therapy approaches you might take to assess, diagnose, and treat the patient’s health needs.
Case: An elderly widow who just lost her spouse.
Subjective: A patient presents to your primary care office today with chief complaint of insomnia. Patient is 75 YO with PMH of DM, HTN, and MDD. Her husband of 41 years passed away 10 months ago. Since then, she states her depression has gotten worse as well as her sleep habits. The patient has no previous history of depression prior to her husband’s death. She is awake, alert, and oriented x3. Patient normally sees PCP once or twice a year. Patient denies any suicidal ideations. Patient arrived at the office today by private vehicle. Patient currently takes the following medications:
Metformin 500mg BID
Januvia 100mg daily
Losartan 100mg daily
HCTZ 25mg daily
Sertraline 100mg daily
Current weight: 88 kg
Current height: 64 inches
Temp: 98.6 degrees F
BP: 132/86
By Day 3 of Week 7
Post a response to each of the following:
List three questions you might ask the patient if she were in your office. Provide a rationale for why you might ask these questions.
Identify people in the patient’s life you would need to speak to or get feedback from to further assess the patient’s situation. Include specific questions you might ask these people and why.
Explain what, if any, physical exams, and diagnostic tests would be appropriate for the patient and how the results would be used.
List a differential diagnosis for the patient. Identify the one that you think is most likely and explain why.
List two pharmacologic agents and their dosing that would be appropriate for the patient’s antidepressant therapy based on pharmacokinetics and pharmacodynamics. From a mechanism of action perspective, provide a rationale for why you might choose one agent over the other.
For the drug therapy you select, identify any contraindications to use or alterations in dosing that may need to be considered based on ethical prescribing or decision-making. Discuss why the contraindication/alteration you identify exists. That is, what would be problematic with the use of this drug in individuals based on ethical prescribing guidelines or decision-making?
Include any “check points” (i.e., follow-up data at Week 4, 8, 12, etc.), and indicate any therapeutic changes that you might make based on possible outcomes that may happen given your treatment options chosen.
